Media Credential Request
To streamline the pass request for sporting events at Creekside High School, each media outlet is asked to submit request for credentials to each game 7 days prior to event. Failure to fill out below sections completely will result in rejection of request. Media Credentials will be issued to an INDIVIDUAL and not GROUPS.
